Credit Freeze
Citizens of Utah can take advantage of the protection of a Consumer Credit Freeze to prevent identity theft. A Credit Freeze simply means that new credit accounts will not be approved and your credit file cannot be accessed by anyone without your approval. Therefore, even if thieves have all of your personal identifying information, they still won’t be allowed to get credit in your name. However, freezing your credit account may slow down the process of obtaining credit legitimately. Fortunately, Utah’s law (S.B. 71S3) includes a provision that requires the credit reporting agencies to thaw your freeze in fifteen minutes. Utah's Credit Freeze Legislation went into effect on September 1, 2008 which now allows for consumers to freeze their credit reports and thaw the freeze in fifteen minutes.
To secure your credit file with a security freeze you must submit a written request to each of the three major credit reporting agencies. Your written request must include the following information:
- Name
- Address
- Date of Birth
- Social Security Number
- Proof of current address such as a copy of a utility or phone bill
- Send a photocopy of your state issued driver’s license or identification card

Victims of identity theft must include a copy of a police report, identity theft report, or other government law enforcement agency report.
Note: If you are a victim of identity theft and provide a copy of a valid identity theft report, or similar proof that you are a victim of identity theft, you will not be charged for the security freeze service.
Required fees:
$10.00 fee to each credit reporting agency to add a freeze
$10.00 fee to each credit reporting agency to temporarily thaw a freeze
$10.00 fee to each credit reporting agency to remove a freeze
The credit reporting agencies will accept a personal check, American Express, Mastercard, VISA, and Discover Card for payment of fees.
If you are paying by credit card, please include the following information:
- Name of the person as it appears on the credit card
- Type of credit card (American Express, Matercard, VISA, or Discover Card)
- Complete card number
- Expiration date (month/year)
For American Express, please include the 4 digit card Identification Number (on front of card above the account number).
For Mastercard, VISA, or Discover Card, please include the 3 digit Card Verification Value Code (CVV/CVC) Identification number (on back of card at the end of the account number).

Temporarily lift a security freeze
You must submit all of the following along with the required personal ID information to each credit reporting agency:
- 10 digit personal identification number (PIN)
- Date range, (e.g. March 15 - March 21) for the temporary lifting of the security freeze, or
proper information regarding the third party to receive your credit report (e.g. Sears).
Permanently Remove a Security Freeze
To permanently remove a security freeze, you must submit all of the following along with the required personal ID information to each credit reporting agency:
- 10 digit personal identification number (PIN)
- Two (2) forms of identification (e.g. pay stub with address and utility bill)
Please return your required personal ID information along with any additionally required information noted above to each credit reporting agency.
