Report ID Theft


Please read the following information before reporting an identity theft crime to law enforcement:

The Identity Theft Reporting Information System (IRIS) is an official State of Utah law enforcement web site from which claims are reported to local, state or federal law enforcement agencies.

Before you start

Identity theft should be reported on this web site by residents of Utah or for crimes that occurred within the State of Utah.
You will need about 20 minutes to complete the process of reporting a crime.

  • You will need the following information:
  • Your Social Security number.
  • Your home address.
  • Your phone number.
  • If known, address of where crime(s) occurred.

    You may need the following information relating to the identity theft crime:

  • Bank and financial documentation.
  • Credit card statements.
  • Credit report.
  • Your driver's license.
  • Utility account records.
  • Phone records.
  • Warrant(s) and legal documentation.
  • Internal Revenue Service records.
  • Government documents.
  • Benefit records.
  • Securities documentation.
  • Letters from creditors.
  • Suspect information.
  • Debt collection letters.
  • Evidence of fraudulent activity.

    If this is your first visit to IRIS, you need to establish a login account before you can file a report. You can create an account by clicking on the "Register Here" link at the left of the login. Once the required information is entered, a validation code is emailed to the address you provide. The email will contain instructions on how to enter the validation code to complete the account set up process.
